Contact Polk Medical Testing Solutions to schedule, order and pay for drug testing services.
Payment must be made with either a credit, debit, or Visa gift card via the phone or eStore. Cash is rarely accepted at any of the drug collection sites.
You must register with Polk Medical Testing Solutions prior to going to the drug collection site. No exceptions.
Bring your registration number and government photo identification (ie. Driver's License or Passport) to the closest drug collection site.
Obtain results via email or fax.
